Person-Centered Planning (PCP)
​Person-centered planning (PCP) is the foundation of the Self-Determination Program (SDP). It outlines the participant’s goals, preferences, strengths, and the support needed to live a fulfilling life.

1. Home and Living Situation​

What type of living arrangement is ideal (alone, with family, with roommates, etc.)?

What support is needed for housing?

2. Health and Well-Being​

Support required for medical appointments, therapies, or personal care?

Mental health and emotional well-being considerations?

3. Employment, Education, and Skills Development
-
Does the participant want to work or learn new skills?

What job training or educational support is needed

4. Social Life and Community Inclusion

What social activities does the participant enjoy?

Are there cultural, religious, or recreational activities to include?

5. Daily Living and Independence

What supports are needed for transportation, cooking, shopping, or budgeting?​​​​​​
6. Identify Strengths and Supports

Strengths: List skills, interests, and abilities.

Challenges: Identify barriers to independence.

Support Needed: Determine what services will help achieve the participant’s goals

7. List the Desired Services and Supports

Based on the goals set in the PCP, list the specific services the participant will need.
