Self-Determination Program (SDP)
1. Develop the Person-Centered Plan (PCP)

The PCP identifies goals, services, and supports needed to transition into SDP.

This plan will be used to determine the Individual Budget for SDP.
2. Budget Calculation & Spending Plan Development

The Regional Center determines the individual’s SDP Budget based on their 12-month spending history.

The individual (with help from an IF, if applicable) creates a Spending Plan to allocate funds for services.
3. Summit Transition Documents and get Approval
-
The IPP Team may revise the budget to accommodate new needs.

Once approved, the individual officially transitions into SDP.
4. Individual Budget and Spending Plan
-
The Regional Center reviews and approves your final budget based on your PCP.

If needed, you can negotiate changes to ensure it meets your goals.
5. Create Your Spending Plan

Include service categories, provider names, costs, and frequency of use.

An Independent Facilitator or Financial Management Service (FMS) can assist in drafting this.
6. Choose a Financial Management Service (FMS)
-
The FMS handles paying service providers, tracking expenses, and ensuring compliance.

The FMS fee is paid from your budget, so make sure to account for this in your Spending Plan

The FMS will require contracts, agreements, and payment instructions before services start.
7. Select and Hire Service Providers

SDP allows you to hire almost anyone, including vendors, individuals, or community resources.

You are not restricted to Regional Center vendorized providers.

You have control over who you hire and how much you pay
8. Begin Services & Monitor Progress

The FMS handles all payments to service providers.
-
Monitor your budget regularly to ensure funds last throughout the year.
-
The PCP and Spending Plan can be updated at any time to reflect changing needs.

If necessary, you can request budget adjustments from the Regional Center.

SDP participants review their plan annually to adjust services and budget for the next year.
